Johnathan died in the fire, Joseph's soul left him in "The World" arc, Jotaro and Josuke die multiple times in the "Bites the Dust" arc, Giorno's body dies in the "Silver Chariot Requiem" arc and Jolyne's soul is replaced in the "Wonderful World" arc.
I'm pretty sure Johnny dies in another universe, and both of the components used to birth Gappy have died in the process.
